Qualification – Registered Nurse or Health Care Professional
Responsibilities – To case manage litigation and settled cases. Assessment, planning, delivering and reviewing Case Management and Therapy plans. Commission and monitor Therapy Services. Recruit and manage Care Teams. Complete and review care plans and risk assessment. Supervise and Appraise staff.
Qualification – Health and Social Care Diploma Level 3+ or equivalent
Responsibilities – To support the Case Manager is assessment, planning, delivering and reviewing packages of care. Recruit and manage Care Teams. Complete and review care plans and risk assessment. Supervise and Appraise staff. To support the Registered Manager in quality assurance activities and clinical governance.
Qualification or Relevant Experience – Business, Finance and Administration
Responsibilities – To support operational activities within the company. Complete administrative duties such as answering telephones, typing, scanning, filing. Support finance activities such as invoicing and purchase orders.
Qualification – Health and Social Care Diploma Level 3+ or equivalent
Responsibilities – To provide leadership to the team. To provide care in accordance with best practice and in line with the company policies and procedures. To support the case manager in training and supervision of the care team. To maintain skills and engage in ongoing learning and development.
Qualification – Care Certificate. Health and Social Care Diploma Level 2+ or equivalent
