At Rebuild 4 Life we build a team around the Client. We co-ordinate with a multi-disciplinary team of professionals who follow the Client through their rehabilitation journey, maximising their potential, and supporting them to manage the long-term effects of their injury to maintain and improve the client’s quality of life, function, independence, social and vocational opportunities.
Case Management is: “A collaborative process which assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet an individual’s health and social needs using communications and available resources to promote quality, cost effective outcomes.” Case Management Society of America.
The role of the Rebuild 4 Life case manager includes assessing the client’s needs and supporting clients to recruit and manage a bespoke support package, procure services from various care and therapy providers. We monitor the service provision ensuring quality and cost effectiveness. This is a collaborative process, which involves establishing a therapeutic working relationship with the client and significant others.
At Rebuild 4 Life all our Case Managers are Registered Health Professionals and Advanced Registered Practitioner with The British Association of Brain injury and Complex Case Management (BABICM).
