Overview

Are you looking for a rewarding role where you can make a genuine difference to someone's life every day?

We are seeking dedicated, motivated and person-centred Rehabilitation Support Workers to join a specialist team supporting a gentleman with an acquired brain injury who lives independently in his own home in Hazel Grove. He is supported by a 2:1, 24-hours-a-day care package designed to maximise his independence, choice and quality of life.

This is not a traditional care role. We are looking for individuals who can build a positive working relationship, support rehabilitation goals, and help our client lead the active and fulfilling life he chooses.

Our client has acquired brain injury-related physical and cognitive impairments but remains very much at the centre of all decision-making regarding his care and support.

He communicates using AAC (Augmentative and Alternative Communication) and is able to direct his own care. He has an excellent sense of humour, enjoys meeting new people and values support workers who take the time to get to know him and learn his unique communication style.

He leads a busy and active lifestyle and regularly attends hydrotherapy, physiotherapy, personal training sessions, community activities, shopping trips, family visits, meals and social outings, including trips to the pub. He also enjoys travelling and regularly holidays with his support team, including activity holidays and trips abroad.

His hobbies and interests include fishing, motorbikes, music festivals, bingo, community events and social activities.

About the Role:

As a Rehabilitation Support Worker, you will work as part of a specialist multidisciplinary team to support our client to achieve his rehabilitation goals and maintain his independence. Your responsibilities will include:

• Providing person-centred support in accordance with best practices, legislative requirements, and agreed standards, under the direction of the Case Manager / Care Coordinator.
• Supporting communication through AAC systems
• Supporting the client with all aspects of daily living, including personal care, manual handling, medication administration, and communication.
• Supporting attendance at therapy sessions and rehabilitation programmes
• Driving and accompanying the client to appointments, activities and holidays
• Encouraging participation in meaningful activities and hobbies
• Supporting emotional regulation and wellbeing
• Providing positive encouragement and motivation
• Promoting independence, choice, dignity and empowerment at all times
• Collaboration with families and therapists to achieve optimal care and rehabilitation outcomes.
• Monitoring and recording the client’s health and rehabilitation progress using electronic care records and EMAR systems.
