Job Description:
Overview
We are seeking motivated, dedicated, and enthusiastic Support Workers to join a specialist rehabilitation support team, supporting a young woman living with the long-term effects of an acquired brain injury. If you're passionate about making a meaningful difference in someone's life, this is an excellent opportunity to deliver high-quality, person-centred support within a rewarding and dynamic environment.
Our client lives in her own home and leads a full and active lifestyle. She enjoys spending time in the community, socialising with friends and family, shopping, eating out, attending appointments, and taking part in a range of leisure activities. She is able to direct her own care and enjoys being actively involved in decisions about her daily life, support, and future goals.
As part of her ongoing rehabilitation, she is supported by a multidisciplinary team of therapists and professionals. Support workers play an important role in helping her engage with therapeutic programmes and implement rehabilitation strategies within everyday activities, helping her maximise her independence, confidence, and quality of life. She also has a much-loved dog who is an important part of her daily life, so applicants must be comfortable around dogs and enjoy working in a dog-friendly environment.
You will work as part of a specialist team supporting our client's daily care, rehabilitation and ongoing development, working closely alongside her family and therapists to deliver high-quality, person-centred support.
Duties will include:
- Providing care in accordance with best practices, legislative requirements, and agreed standards, under the direction of the Case Manager, Nurse and Family.
- Providing all aspects of personal care with dignity and respect
- Supporting moving and handling activities
- Administering medication as prescribed
- Supporting PEG feeding requirements
- Supporting communication through AAC and recognising individual communication cues
- Delivering therapy and rehabilitation programmes as directed by the multidisciplinary team
- Supporting participation in community activities and meaningful experiences
- Promoting independence, choice and inclusion wherever possible
- Working collaboratively with family members and healthcare professionals to achieve optimal care and rehabilitation outcomes
- Maintaining accurate records and contributing to goal-focused rehabilitation plans using electronic care records and EMAR systems

- Ongoing supervision and support
- The chance to work in a nurturing and collaborative environment focused on making a genuine impact.
- The opportunity to make a lasting difference to a young person's life

- Have a caring, patient, and dedicated individual with a fun and gentle approach
- Are patient and willing to take time to build relationships
- Are flexibility and adaptable to meet the evolving needs of our client.
- Have excellent communication and observational skills
